Do I need a referral?

It depends on your plan. HMO plans usually require a referral from your primary-care physician; PPO and POS plans usually don't. If your plan requires a referral and you don't have one, we can still see you on a self-pay basis, then help you obtain a referral for follow-up. For DermNow walk-in visits, no referral is required.

Will my insurance cover cosmetic services?

No — cosmetic dermatology is not covered by insurance. Cosmetic services are billed as cash-pay, with prices discussed before treatment.

Are biopsies and skin-cancer treatments covered?

Yes — medically necessary biopsies, excisions, and skin-cancer treatments are covered by insurance. Cosmetic mole removal (for appearance only, no medical concern) is not covered.

What if my insurance isn't on your list?

Call us at (412) 659-0599. We add new plans regularly, and we can verify whether your specific plan is accepted before you come in.

Do you accept Medicare and Medicaid?

Yes — Monument accepts both Medicare and Medicaid.

What about HSA / FSA?

HSA and FSA cards are accepted for medically necessary visits, copays, and prescriptions. They are not accepted for cosmetic services.

I have insurance but want to pay cash. Is that an option?

Yes. Cash-pay rates are posted on the cash pay page. Some patients choose this for visits below their deductible.

When are payments due?

Copays and known coinsurance are collected at the time of service. Balances after insurance processing are billed separately within 30–60 days of the visit.
